Northern Arizona loves Arizona wines and distilled spirits! If you’d like to introduce your products to this unique market, we’d love to have you! Please fill out the following registration information in order to attend Off the Rails | A Route 66 Wine Festival:

  1. Participation Agreement
  2. City of Kingman Business License or City of Kingman Special Event Vendor Permit plus:
    1. $11 check payable to City of Kingman
  3. Certificates of Liability Insurance and Endorsements
    1. Limits required:
      $1,000,000 each occurrence, $2,000,000 aggregate, $100,000 rental damage; shall include bodily injury, property liability and $1,000,000 products/completed operations coverage limits. Medical (any one person) = $5,000.
    2. Description of Event:
      1. Off the Rails | A Route 66 Wine Fesitval
      2. Saturday, Oct 14, 2017 11:00am-6:00pm and and Sunday, Oct 15, 2017 11:00am-4:00pm
      3. Locomotive Park, 310 Beale St.., Kingman, Arizona 86401
      4. Certificate Holders:
        1. Historic Route 66 Association of Arizona, 120 W. Andy Devine, Kingman, AZ 86401
        2. City of Kingman, 310 N. Fourth Street, Kingman, AZ 86401
        3. Ignite Brand Marketing, 7916 North Paradise Canyon Lane, Prescott Valley, AZ 86513
      5. Include a copy of the actual “Additional Insured” Policy endorsement that meets above requirements, which must include policy number, “City of Kingman”, “Historic Route 66 Association of Arizona”, and “Ignite Brand Marketing” under schedule.
  4. $100 check, made payable to: Ignite Brand Marketing
